Job type: Permanent. x1 Full time. 37.5 hours per week. Monday - Friday and x1 part time, 22 hours per week, Monday - Friday
Salary: Up to £27,597 WTE (pro-rata to 22 hours).
Main duties of the job
Care Coordinators provide extra time, capacity, and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals. They will support the Clinical directors in coordinating all key activity including improving access to services, providing advice and information and ensuring health and care planning is timely, efficient and patient-centered. They will work closely and in partnership with the Social Prescribing Link Workers and support delivery of the comprehensive model of care to reflect local priorities, health inequalities or population health management.

Job responsibilities
Management of medical records and clinical correspondence:
* Ensure that patient records are accurately maintained.
* Scanning of clinical correspondence into our clinical systems.
* Ensure that clinical correspondence is received and processed in an accurate and timely manner through the Docman system.
* Active document management, reviewing clinical correspondence before electronically workflow to the GPs.
* Ensure correspondence, reports, results etc. are filed in records.
Screening and Recalls:
* Ensure familiarity with all appointment systems including regular and incidental variations.
* Book appointments and recalls ensuring sufficient information is recorded to enable retrieval of the medical record.
* Proactively contact patients to invite them for cancer screening programmes, immunisations and long-term condition reviews.
Administrative support:
Provide administrative support to members of the primary healthcare team in the following areas ensuring appropriate practice records are kept up to date:
* Cytology (smears)
* Seasonal and targeted Immunisation recalls
* Private medical insurance reports
Creation of patient searches (in EMIS, our clinical system) to support the coordination of project work across the hub. Able to build relationships with ARRS staff, service users and allied professionals in member practices; to help identify patients who could be referred to other health care professionals within the PCN to help improve PCN efficiencies.
Provide coordination and navigation to patients across the PCN (Primary Care Network). Help people to manage their needs through answering queries, signposting to available services, and ensuring that people have good quality written or verbal information to help them make choices about their care.
Support delivery of the comprehensive model of care to reflect local priorities, health inequalities or population health management. Holistically bring together all of a person's identified care and support needs, and explore options to meet these using all available services and resources.
