Medication Management : Administer prescribed medications and treatments in line with care plans, ensuring accuracy and safety. Collaborative Care : Work closely with multi-disciplinary teams to deliver holistic, person-centred care that addresses both physical and mental health needs. Family Support & Education : Provide support and guidance to patients and their families, educating them about mental health conditions and treatment options to promote understanding and recovery. Monitoring & Evaluation : Continuously monitor patient progress, adjust care plans accordingly, and provide ongoing support to ensure the best possible outcomes.
