You will work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients, using your expert knowledge of medicines for specific disease areas; 2. You must be enrolled in, or planning to enroll from an approved 18-month training pathway or equivalent that equips you to be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE Clinical Pharmacist training pathways) this is if you are not already a prescriber. 3. You will support patients through structured medication reviews to take their medications to get the best from them, reduce waste and promote self-care; 4. You will demonstrate safe and effective clinical decision-making and expert care on a day-to-day basis through triage, assessment, and diagnostic skills, both remotely and face-to-face; 5. As well as dealing with acute illness (outlined above), you may be involved in the management of long-term or chronic conditions, specifically through undertaking clinical medication reviews to proactively manage complex polypharmacy; 6. You will provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of our patients, and to help in tackling inequalities; 7. You will work with Trafford PCN to provide support to the Limelight clinical team with regards to person-centred medicines optimisation and quality improvement, whilst contributing to the Quality and Outcomes Framework and Enhanced Services; and 8. You will develop relationships and work closely with other pharmacy professionals and the wider health and social care system.
