Responsibilities:
2 x Occupational Therapists required to work within busy Locality Team (Urban) required to provide cover for a Project Team, to offer operational resilience and address community based waiting lists for assessment and provision of supports and equipment etc.
Posts generally office based although flexibility is required to accommodate local home working arrangements currently in place (Covid 19); workers must also be available to undertake essential visits within domestic addresses/ individual properties as required.
2 x Occupational Therapists required to work within busy Hospital Discharge Team (Re-ablement and Intermediate Care Service) - Posts required to provide additional cover, to offer operational resilience to promote successful discharges and mitigate delays from Acute and Intermediate care services.
Posts generally office based although flexibility is required to accommodate local home working arrangements currently in place (Covid 19); workers must also be available to undertake essential visits within Acute/Intermediate Care settings as required.
Locums will undertake a range of tasks including assessments and interventions as related to their role.
1 X Care Manager (OT) - potential requirement for back fill within the service.
Requirements:
All candidates must:
1. have a degree in Occupational Therapy
2. be registered with the HCPC
3. have demonstrable experience in Re-ablement practices and processes
4. Awareness of and current/recent training in: Adult Protection - Practice and Process *
5. have knowledge and proficiency to support discharge from hospital/ Intermediate Care
6. hold full driving licence
7. Knowledge and experience of SWIFT - desirable
*preferred, but not essential
