We are a very busy Town Centre Practice covering 3 sites and growing rapidly! We are looking to recruit highly motivated, enthusiastic and versatile staff for our very busy practice. The Care Navigator will work in a reception office navigating our patients via telephone or face to face. Please note this is a very busy practice and we require someone to work hard to meet the requirements of the patient and team.
We are looking for applicants to sell themselves in this application. If successfully shortlisted we would expect applicants to have done some research into the practice and role. We hope to recruit someone who can grow into the role and become a long-term dedicated member of the team.
Main duties of the job
The post requires you to be flexible, responding to varying demands and changing circumstances. Communication, teamwork, motivated hard work ethic and a sense of humour are essential qualities to succeed in this role! Previous experience in a healthcare reception office is desirable; however, full training will be given. Please note this is a reception office-based role. Flexibility to cover additional hours for holidays and sickness is essential.

Hours will range between 8am and 6.30pm Monday to Friday AND 9am-12pm Saturdays. Your hours will be split between Monday to Friday, and they will include EVERY Saturday morning 9am-12pm.

JOB TITLE: Care Navigator
DEPARTMENT: Reception
Reports to: Workforce Administrator/Management Team
Job Summary:
Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.
Provide general assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.
Job Responsibilities:
Administration
* To have a thorough knowledge of all Practice procedures.
* To work in accordance with written protocols.
* Scan relevant documents into patient records.
Reception
* Receiving patients consulting with members of the Practice team, directing to self-arrival where appropriate.
* Promote services to patients, i.e., online registration, patient partner, texting, Accurx.
Appointments
* Process appointment requests for appointments from patients by telephone, online and in person.
* Deal with visits requests.
Computer
* Registrations of new patients.
* Process repeat prescription requests in accordance with Practice guidelines.
Telephone
* Use headset when receiving or making outgoing calls in accordance with the Practice's health and safety guidelines.
* Answer all calls promptly using the practice script.
* Deal with queries as per the practice's protocols.
Other Tasks
* Check surgeries at the end of the day as per practice protocols.
* Ensure building security is maintained.
* Any other tasks allocated by the Administrator Team and Management Team.

Person Specification
Qualifications
* Good educational background.
* A-C grade or equivalent in English and Maths.
