Senior Clinical Pharmacist IP - London Area (REF: TPN-SNR-CP-IP-LON)
The right candidate for the role will have completed their IP qualification and have completed their CPPE pathway. The right candidate for the role will have at least 1 year experience in primary care in a leadership role.
An exciting opportunity has arisen for a clinically focused, highly motivated clinical pharmacist who wishes to expand their experience by working remotely for General Practices within the London Area.
Main Duties of the Job
Using GP systems (EMIS and System 1), you will support practice sites with medicines-related tasks. This may include, but is not limited to:
* Supporting patient requests for acute and repeat medications
* Structured medication reviews
* Safety audits

Job Responsibilities
You will work as part of a multi-disciplinary team in a patient-facing role, working with colleagues to help resolve medication-related issues, utilizing skill mix, improving patient outcomes, and ensuring better access to healthcare.
You will support practice staff with prescription and medication queries, help support the repeat prescription system, deal with acute prescription requests, and conduct medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.
You will support prescribing practices across the PCN, providing clinical leadership on medicines optimization and quality improvement.
