BHRUT is looking for two motivated Child Health Hub MDT Coordinators to join our dedicated team. If you are passionate about improving child health services and enjoy working in a collaborative environment, this is the role for you!
As a Child Health Hub MDT Coordinator, you will play a vital role in supporting the multidisciplinary team (MDT) within the Primary Care Network (PCN). You will be responsible for coordinating the efforts of health professionals such as GPs, social care workers, therapists, and voluntary organisations to ensure seamless care for children and families across Redbridge and Havering.
Your role will include supporting the team to implement actions, promoting effective communication, and ensuring a smooth flow of information between practitioners. You will also liaise with patients, families, and practice champions to promote engagement and ensure services meet their needs.
Key Responsibilities:
1. Support the coordination of the MDT, facilitating communication and collaboration.
2. Act as the primary point of contact for team members.
3. Assist in promoting patient engagement and co-production with families.
4. Track and report on the effectiveness of Child Health Hub services.
5. Contribute to continuous improvement efforts within the team.

Our patients are benefitting from our Women’s Health Hub in Ilford; an Ageing Well Centre in Hornchurch; and Community Diagnostic Centres (CDC) at Barking Community Hospital and at St George’s Health and Wellbeing Hub in Hornchurch. These CDCs are open 12 hours a day, 7 days a week and are providing an extra 88,000 scans a year.
The majority of our 8,000 staff – who come from 146 different countries - live in the three diverse London boroughs we serve and are from black, Asian and minority ethnic groups. Many can work flexibly and more than 400 of them are on our Ofsted accredited apprenticeship programmes. We’re proud to be a London Living Wage employer.
