Job summary The Acquired Brain Injury Team are delighted to offer an exciting opportunity for a Team Lead & Case Manager to join our community specialist team and support the delivery of neurorehabilitation to patients living with an acquired brain injury in the Community. The Acquired Brain Injury Team provide specialist assessment, rehabilitation and advice for patients, families and carers. We are looking for a suitably qualified, experience and registered professional from a field such as social work, nursing, occupational therapy, physiotherapy, speech and language therapy to fulfil this role. The team have an administrative base in Carlisle and Kendal and covers the whole of Cumbria so the successful candidate must be prepared to travel as part of their job role. As a Team Lead the post holder will provide clear direction, support and guidance to the Acquired Brain Injury Service by adhering to, and the implementation of service plans Trust Polices and Standard Operating Procedures. As a Case Manager the post holder will lead on new referrals and assessment of patients into the service. The Case Manager will work collaboratively with the interdisciplinary team to design and implement specialist rehabilitation plans. Main duties of the job To provide specialist assessment and rehabilitative packages of care to patients with brain injury aimed at reaching the clients full potential. To work with other health, social care, voluntary agencies including out of area establishments regarding the repatriation of patients. The service is considered locally and nationally as an exemplar service, offering evidence based intervention for adults, primarily of working age with an acquired brain injury, including trauma, stroke, anoxia, inflammation etc. The post holder will become part of a team providing neurorehabilitation to patients living with an acquired brain injury in the community. They will work directly and indirectly, providing: Direct care for complex cases, working with patients and their families. Consultancy to teams to support the case management of patients care and treatment. Supervision of other professionals, including Occupational Therapist, Physiotherapist, Speech and Language Therapist and Rehabilitation Assistants. Contribution to continued professional development of other Allied Health Professionals within the service. Providing group interventions alongside other Allied Health Professionals. To lead on the coordination of new referrals into the service, ensuring assessments are carried out in a timely manner. To monitor and evaluate the goal -orientated process for all patients able to participate in identifying and setting their own goals collaboratively with the team and the patient.
