Job Summary
Are you an experienced qualified nurse who enjoys working in partnership with patients and their families? We are seeking a passionate and motivated Registered General Nurse to join our Night community nursing teams working in the Great Yarmouth and Waveney area. You will be based at Beccles Hospital and will work with a Health Care Assistant to deliver care to patients across the Great Yarmouth and Waveney areas. The successful candidate will be an experienced practitioner who is able to support patients and their families in challenging situations. As a clinician working out of core hours the role will require an ability to work closely with professionals from other organisations. The night service currently works between the hours of 20.00 and 07.00.
The service delivers unplanned care to patients and will require the successful candidate to be resilient and resourceful. You will be working within an established team of clinicians who visit patients in their own homes and Residential Care, visits are completed by a registered and a non-registered nurse working in partnership. The desire to deliver excellent end of life care is essential.
Main duties of the job
The successful nurse will have competence in assessment of patients, who may be in distress, and proven skills in symptom management within end of life care. Competence in urethral and supra-pubic catheter care is also advantageous. The team respond to urgent community response requests including people who have fallen and care crisis.

The post is for 25 hours per week, 5 X 10 hours shifts over 2 weeks which may be rostered as 5 nights consecutively.

Job Responsibilities
* To work with other members of the multidisciplinary team to strengthen the emerging Out of Hospital System.
* To work autonomously at a level appropriate to clinical competence and within the scope of professional practice.
* To develop links between key partners, Out of Hours GP service, care homes, acute trusts and specialist services within and outside of ECCH.
* To assist others with the continuous evaluation of the service to ensure it is fit for purpose and increases access and choice.
* To provide clinical and professional support to the community nursing teams across ECCH localities.
KEY RESULT AREAS
Role Specific
* Act as a resource for health care professionals on all matters relating to community nursing.
* Plan and organise a range of complex activities in a wide range of settings to ensure best practice is delivered across your designated area of responsibility and the wider community.
* Work on a rota basis as member of a multidisciplinary team providing access to services on a 24/7 basis.
* Support the leadership and management of community nursing teams.
* Work collaboratively with residential home colleagues to promote high standards of healthcare.
* Participate in teaching and clinical supervision of support workers and other provider staff as required.
* Contribute to the clinical governance agenda through participation in clinical risk assessment and management, clinical audit.
* Work with colleagues to develop care pathways for patients with a range of healthcare needs.
* Develop excellent internal and external networks in order to promote the community nursing service.
* Undertake continuous professional developments as identified through appraisal and personal development planning.
* Work with ECCH Colleagues, and other partner agencies and stakeholders including the acute trust to contribute to the development and delivery of new innovative models of service delivery.
* Contribute to the review of current policies/protocols/guidelines and the development of clinical pathways and outcomes for this client group.
* Critically evaluate research findings, national guidelines and implement changes in clinical practice as appropriate.
* Create an environment conducive to effective working, respecting and supporting staff to deliver high-quality clinical services.
* Ensure staff compliance with ECCH risk Management systems, proactively identifying risk within specialist services and ensuring actions to reduce and manage risk.
* Ensure a high standard of record keeping is achieved in line with ECCH and professional standards.
* Contribute to, support and promote ECCH's strategic direction, values and culture in relation to community nursing.
* Make positive contributions to excellent communications across the organisation.
* Represent the Community nursing service at external bodies as required.
* Take responsibility to ensure compliance with Health and Safety Policy, Fire and Environmental Waste Regulations.
* Ensure own compliance with the Nursing and Midwifery Codes of Conduct and Competence and the NHS Managers Code of Conduct.
