We have full and part-time opportunities in our Home First Hub, which is an integrated team working closely with our Norfolk and Community Care (MCH&C) and NHS colleagues to ensure safe and timely hospital discharge for those in community and acute hospitals across the Central Area of Norfolk.
This is a new, developing team committed to supporting people to return home, access reablement opportunities and plan their ongoing care and support needs once discharged.
As a Supported Assistant Practitioner (Scale F), you would join our 9-month, bespoke support programme, enabling you to progress to an Assistant Practitioner role (Scale G) subject to satisfactory programme completion.
You will possess experience in the Health or Social Care sector, have a good standard of education along with excellent IT and organisational skills. You will be competent at working at pace in a responsive fast-paced environment, with a positive can-do attitude. You will use your professional curiosity around information exchanging and community resources, possessing the ability to work independently or as part of a team.
Along with good listening and analytical skills, you will be flexible and adaptable to new ways of working and committed to supporting people to leave hospital safely. In return, we will offer a well-planned induction, the opportunity to learn new skills and a supportive working environment while making a difference to the communities of Norfolk you work with.
This is a hybrid role. You will be required to visit hospitals, complete face-to-face assessments as well as attending your office base at least twice a week but also have the opportunity to work remotely for some of the time, so require good broadband connectivity. As part of a large-scale transformation in support of the Discharge to Assess model and to optimise the experience for our service recipients/patients, your role may stay in The Hub or move to assessing people in their own homes.
Some weekend working will be required.
