Sherwood Forest Hospitals NHS Foundation Trust is seeking a dedicated and proactive discharge specialist practitioner to join our team, focusing on discharge coordination. The successful candidate will play a pivotal role in ensuring the timely and effective discharge of patients across the Trust. Working closely with ward-based multi-disciplinary teams and the discharge hub, you will:
• Ensure the provision of specialist advice.
• Develop and implement policies to manage the discharge process.
• Ensure all patients with long lengths of stay over 14 days have clear discharge plans.
• Conduct Reason to Reside checks for all patients.
• Participate in the Length of Stay (LOS) report weekly meetings and other relevant meetings.
