Job summary This is an exciting opportunity to improve the young person's experience of transition and that of their families by developing partnerships with them, along with the professionals from adult and paediatric specialities. You will be required to develop processes that are cost effective, safe and sustainable and support health professionals in delivering developmentally appropriate healthcare for young people. This will involve mapping the current transitional pathways for Children and Young people for each speciality and implement a programme of gap analysis (using national guidance) and support where such pathways are not in place, with final report to divisional board to take forward actions that are beyond the remit of the transition team. Main duties of the job Build working relationships with professionals from both Children's and Adult services for effective transitional pathway development and provide support to healthcare professionals when transitional arrangements are complex. You will support young people and families by ensuring their transition occurs in a cohesive and a coordinated manner and there is effective liaison with central transition teams in adult services to facilitate transition and problem solve barriers jointly. Develop and provide suitable transition and adolescent health training packages for various staff groups with the Trust and ensure all specialities have transitional pathways which are developed jointly with adult services. Ensure the trust's transition related policies and pathways are up to date and implement national guidance as appropriate.
