You will join an established team of clinicians providing nursing interventions for patients with a diagnosis of learning disability with a range of complex health needs including physical health, mental health and behaviour. You will be part of a community nursing team working within the wider learning disabilities multi-disciplinary team.
We are looking for experienced clinicians to take the challenge of delivering a high standard of nursing intervention within the learning disability service. Do you have the skills and sound clinical knowledge to manage a caseload of patients and provide robust support and clinical leadership to your nursing and multi-disciplinary team colleagues? Are you a dedicated and motivated individual with experience and the ability to work independently? We are looking for organised and efficient clinicians with good communication skills to work within this busy team. If you enjoy variety and a fast pace of work within a patient focused setting, this could be the job for you.
Main duties of the job
1. To work within the framework of a Multi-disciplinary Team to provide specialist support and guidance to adults with learning disabilities and their carers.
2. To provide a clinical service through the effective management of an active caseload.
3. To assist the Team Leader in the provision of leadership to the Community Nurses.
