Job summary We are looking for an enthusiastic and suitably qualified and experienced individual to join our friendly Intermediate Care Therapy Team. We have a full time post (37.5 hours) to be worked Monday - Friday but this could change to a 7 day week in the future. The post holder will be based at Leigh Town Hall and work as an integral part of the Intermediate Care Therapy Team providing an in reach therapy service to patients within the Intermediate care and Community beds within the borough. It is essential for this role that you have the ability to independently travel across the borough as part of the role involves carrying out assessments in patients' homes. You will carry out assessments, formulate therapy plans and deliver therapy interventions for patients in their own place of residence. NHS experience is essential. Main duties of the job The post holder is required to work as part of the Therapy team as an autonomous practitioner. To assess and undertake agreed treatment programmes with individual patients underpinned by therapy specific knowledge (i.e. Occupational and Physiotherapy). To undertake environmental visits and follow up visits as required. To maintain clinical records as per Trust policy and professional guidelines. To carry out administrative duties, in line with service needs. To manage and maintain a therapy specific caseload with support and oversight from qualified therapy staff.
