Full Time - Primary Care Business Change Facilitator - Band 6

As part of our Primary Care Digital Programme, you'll play a crucial role in bringing the national vision for integrated primary care to life. You'll work closely with a diverse team on exciting IT and digital projects that directly benefit our local community. You'll be at the forefront of transformation, supporting General Practice, Dentists, Optometrists, and Community Pharmacies as they embrace new digital tools and ways of working.
Main duties of the job
* Be a digital champion! You'll design and deliver top-notch training programs to help primary care staff make the most of their IT systems (like EMIS, SystmOne, Docman, and Accurx).
* Bring people together! You'll facilitate communication between practices, encouraging idea-sharing and collaboration to boost digital transformation.
* Make change happen! You'll work with project managers to articulate the vision and benefits of new initiatives to primary care teams.
* Be the go-to tech expert! You'll provide valuable technical knowledge to support the Primary Care Commissioning and Development Team.
* Keep things running smoothly! You'll help identify and address potential roadblocks to ensure the success of our digital change programs.
* Bridge the gap! You'll act as an intermediary, resolving conflicts and ensuring teams have the resources they need to embrace new ways of working.
* Be a problem-solver! You'll analyse current processes and help design improved workflows that benefit both staff and patients.
* Stay on top of it all! You'll manage project risks, track progress, and keep stakeholders in the loop.

Person Specification
Knowledge, Training and Experience
* Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
* Evidence of success in efficient and effective change / transformation project and programme implementation in recent role
* Extensive, recent experience of working with primary care clinical and other IT/digital systems (e.g. EMIS/S1, Docman, Accurx)
* Project Management Experience and qualification e.g. Agile or equivalent
* Solid understanding of Change Management and improvement processes
* Knowledge of Caldicott Principles, Information Governance and Freedom of Information and the implications for electronic records
* Specialist knowledge and experience of mobilising change implementation / transformation projects in general practice

* Possess a high degree of accuracy and attention to detail, with an ability to analyse and interpret complex facts or situations
* Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making
* Experience of setting up and implementing internal processes and procedures

* Clear communicator with excellent training, writing, report writing skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences.
* Present, articulate and demonstrate information to a large and often high -level audience in a manner that fosters engagement and stakeholder buy -in
* Good agile project management skills with the ability to work on multiple projects at the same time.

* Previous experience in project management and planning.
Physical Skills
* Good knowledge of Microsoft Office proficient in Excel, PowerPoint, Teams and Word.
