The Cardiac Surgery Nurse Case Managers are looking for an experienced senior nurse to work within their team. The post will involve clerking patients in preoperative clinics, coordination of cardiac theatre lists, liaising with regional hospitals and ensuring appropriate and timely transfer and scheduling of inpatients, MDT and MDT working, autonomously manage surgical waiting list and being a main contact for preoperative patients.

WHAT YOU’LL DO
The Nurse Case Coordinator is responsible for supporting the Nurse Case Manager in leading nurse-led treatment and care for a defined caseload of patients, where they practice as an autonomous clinician within the multi-professional team.
The post carries responsibility for the safe delivery of a number of defined roles not routinely performed by nurses within this speciality, and requires competence in specific areas of knowledge, technical skill, nursing expertise and clinical decision-making.
The post carries responsibility for ensuring effective and efficient management of the cardiac surgery waiting lists including appropriate scheduling to ensure that the theatre and critical care resource is used to its maximum and meeting government waiting time targets and local delivery plans.
To provide specialist expertise, advice, education and support to patients, their carers and other health care professionals, and advise and support the Cardiovascular & Thoracic Care Group regarding the management of the service. In addition, the post holder will work across professional, departmental and Trust boundaries as required to develop and support patient-centred seamless care.
