Walsall West One PCN has a full-time vacancy for a Pharmacy Technician. We are a dynamic Primary Care Network of six passionate General Practices within the West of Walsall. We have a dedicated, enthusiastic team of health care professionals comprising different skills and specialties including Physiotherapists, Paramedics, a Pharmacist Technician, Social Prescribers, and a Health Well-Being Coach.

Main Duties of the job:
* The postholder will work as part of Modality Partnership Walsall Division working as part of a pan-divisional pharmacy hub which will include pharmacy administrators, pharmacy technicians & prescribing pharmacists, and who will deal with all pharmacy-related patient inquiries, as well as the delivery of structured medication reviews, high-risk drug monitoring, medicines reconciliation, repeat medication facilitation, drug audits, and other clinical work depending on your competencies.
* The role is not a regular 9am-5pm job; the job requires innovation, flexibility, and commitment and the postholder will be required to work resourcefully as part of the team to ensure tasks are completed.

Job Description:

* Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.
* Carry out medicine’s optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews, and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.
* As determined by the Division, support medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacies.
* Support the Clinical Pharmacist in Structured Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR.
* Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.
* Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.
* Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.
Working under the supervision of the Lead Clinical Pharmacist you will ensure the safe, accurate, and timely supply of prescribed medication to patients and will provide technical and administrative support to the Clinical Pharmacists and Clinicians.

Qualifications and Experience:
Essential:
* GPhC Registered Pharmacy Technician
* BTEC/NVQ level 3 or equivalent in pharmacy services
Desirable:
* Enrolled in, undertaking or qualified from, an approved training pathway. For example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH)
Experience:
Essential:
* 2 years’ experience in a hospital, community, or primary care pharmacy

* Excellent communication skills (written and oral)
* Clear, polite telephone manner
* Excellent IT skills including Microsoft Office packages and case management systems
