The service is actively developing new roles and ways of working, and has developed a stronger interface across hospitals, in the community and within social care services to ensure that people are referred to the most appropriate service in a timely manner. The post holder will be required to keep up to date with local and national strategic drivers in the areas of Care Closer to Home. ll applicants will be required to produce at interview a portfolio evidencing the ability to lead people through change and the evaluation of the change. The portfolio should also include supporting staff in their clinical and professional development and demonstrate their clinical practice in a variety of settings. The post holder will be supported in their clinical and professional development. Funding is available for internal and external training. Supervision, a regular PADR review and ongoing education are an integral part of staff development.
