Job summary Your nursing career - your choice An exciting opportunity has arisen to recruit a Band 7 Discharge Coordinator in the Discharge Team to complement our nursing and admin team. You will be working as part of a multi-professional Team to support the smooth running and co-ordination of discharge services within the unplanned Care Division and preventing unnecessary, avoidable delays for patient discharge. Main duties of the job The post holder will be expected to drive the Trust's discharge process and contribute to the collective performance of the team, ensuring complex discharges are proactively and safely managed within the adult inpatient areas. You will be an RGN with experience in discharge planning and possess a teaching/mentoring qualification or equivalent. The post holder will be responsible for facilitating safe and effective discharges of patients from acute to the community setting through a multidisciplinary approach over 7 days. You will work closely with the clinical teams, supporting and advising on all aspects of complex discharge planning, ensuring all appropriate referrals are completed to local authorities, ICB's etc. and preventing unnecessary, avoidable delays for patient discharge. Co-ordinate assessments of patient's needs and support the discharge plan. The post-holder will be proactive in identifying concerns or issues that may delay effective discharges and work with the Integrated Discharge Team to overcome and limit these, acting as a specialist and demonstrating a high level of expertise within the transfer of care (discharge) service, providing specialist advice, education and support to health care professionals and patients.
