Job Description:
West Lothian offers Housing Support and Care at Home services to Adults and Children, we also deliver in conjunction with West Lothian Council.
Post: Support Worker Service: Housing & Community Support Services (HACSS)
Base: Regional base as set by line manager
Line Managed by: Practice & Wellbeing Leader
Requirements :
Driving License preferred or able to travel around West Lothian on public transport.
Salary :
Casual rate: £12 per hour
Hours: Full-time, Part-time and Casual, hours will be discussed with the post holder.
Main purpose:
The main purpose of this post is to provide a caring, efficient and effective 24/7 support service to people who use The Action Group’s Services. This will include assisting people to develop their skills by supervising and participating in various support activities in a way that enriches their opportunities and promotes choice. The main duties for this post are detailed below under two headings: 1. Specific Duties 2. General Duties.
Specific Duties
1. Practice the TAG Key Worker system, organizing support plans and case reviews as required.
2. Promote the physical and emotional wellbeing of the people we support.
3. Actively develop meaningful and trusting relationships with the people we support.
4. Encourage independence in all daily living activities.
5. Promote people’s hobbies, interests, and activities.
6. Directly assist with personal care as required.
7. Work with the people we support to maximize their ability to communicate effectively.
8. Advocate on behalf of the people we support when needed.
9. Work with relatives and carers of people we support.
10. Find out about local services and organizations available to people we support.
11. Participate flexibly in various shift work patterns.
12. Regularly undertake 'sleepover' or 'waking night' duties.
13. Cover short-term absences of Support Workers in other teams when required.
General Duties
1. Read and work in accordance with TAG’s Mission, Aims & Objectives.
2. Manage own work time effectively.
3. Maintain effective and organized filing systems.
4. Treat everyone with consideration and respect.
5. Ensure prompt, polite, and clear communication.
6. Comply with all applicable TAG policies and procedures.
7. Promote and respect individual rights and responsibilities.
8. Retain the highest standards of confidentiality.
9. Assist your manager and colleagues as appropriate.
10. Take charge of own CPD (Continuous Professional Development).
11. Be responsible for own work-life balance.
