To demonstrate competent and efficient patient assessment care needs, coordinating with the unit link role resources and multidisciplinary team, to ensure patients receive their optimum care :- - to undertake a competent and accurate clinical pre & post dialysis assessment. - to demonstrate a competent and accurate development of patient care plans. - to demonstrate a competent and accurate implementation and evaluation of patient care. - to demonstrate a competent and accurate monthly review of patient clinical data, providing evidence of urea kinetic monitoring and evaluation care to optimise clinical standard achievement. - to demonstrate a competent and accurate monthly review of the patients dialysis prescription, providing evidence of evaluation care to optimise clinical standard achievement. - to demonstrate a competent and accurate review of the patients coagulation management, with activated clotting time monitoring, providing evidence of evaluation care to optimise clinical standard achievement. - to demonstrate a competent and accurate review of the patients anaemia management, providing evidence of evaluation care to optimise clinical standard achievement. - to demonstrate a competent and accurate review of the patients bone disease profile, providing evidence of evaluation care to optimise clinical standard achievement. - to demonstrate a competent and accurate infection control review of the patient and patients access, providing evidence of evaluation care to optimise clinical standard achievement. - to demonstrate accurate and compliant patient assessment documentation. To demonstrate accurate and competent evidence of the holistic dialysis care provided as a named nurse. To maintain an in-depth knowledge of the patients physical, psychological and social state, promoting the patients interests as a nurse advocate :- - to maintain an awareness of all developments in the patients clinical timeline e.g. outpatient appointments, referrals, investigations, multidisciplinary team etc. - to interact with the patient in the planning of their care to optimise empowerment and independence. - to perform documented holistic assessments of the patient at least every 3 months. - to identify problems and potential problems in conjunction with the patient, providing support in developing plans to address any issues. - to competently select the appropriate care pathway for identified problems. - to monitor access management 3 monthly and review results. - to monitor activated clotting time ranges 3 monthly and review anticoagulation. - to seek to interact with the patients family in their care, as per patient consent. - to maintain the patients documentation in a compliant and legible order. - to manage the monitoring of the patients Hepatitis and HIV status as per policy. - to complete, competently and accurately, the patient manual handling assessments as per policy. To promote and maintain professional relationships with both patients and Trust staff to enhance the business excellence and partnership with B Braun Avitum UK Ltd. To work within the Avitum Policies and Procedures and adhere to the Quality Manual and Environmental, Health and Safety Guidelines.
