Job summary An exciting fixed term opportunity has arisen for an experienced Mental Health or Learning Disability Nurse with working knowledge and experience of locked and secure hospitals, alongside essential knowledge of our local Dynamic Support Pathway. The fixed term offers an opportunity to work within a well-established and committed LDA- Admission Avoidance and Discharge team within the NHS Herefordshire and Worcestershire ICB. Working directly with people in crisis and navigating our local Health, Education and Social Care systems together to avoid admission, and overseeing timely discharges from specialist MH hospitals. This role will involve chairing Care, Education and Treatment reviews (CETR) and completing Quality Assurance reviews of locked services commissioned by the ICB across MH and LDA services. Main duties of the job The post holder will require knowledge and experience to review the quality and effectiveness of placements, for adults, children and young people with complex Mental Health and a formal diagnosis of a Learning Disability and/ or Autism, to ensure services are meeting assessed needs. The post holder will need to have a good knowledge and understanding of the LDA admission avoidance pathway and Mental Health Act. Experience of chairing Care, (Education) and Treatment Reviews (C(E)TR), alongside an excellent knowledge of the Dynamic Support Pathway (DSP). They will carry out complex reviews and interventions often in challenging or crisis circumstances for all age group of people, at times when individuals may be presenting with risky behaviors related to their personal situation. NHS Herefordshire and Worcestershire Learning Disability and Autism (LDA) Admission Avoidance formally Transforming Care Programme (TCP), forms part of the systems NHS long term LDA plan to supporting autistic people and people with Learning disability to avoid admissions to inpatients services and step down from locked, CAMHS T4 and secure in- patient services as soon as it is safe to do so. The post holder will be expected to complete Quality Assurance visits to specialist locked hospitals. Please note that the post holder will be required to travel to services across the country to make face to face contact to with individuals they are working with. About us Key Working Relationships Internal: The post holder will maintain and develop working relationships within our integrated care system health, education and social care specialist teams, who provide case management to individuals known to the DSR the LDA admission avoidance team, as well as reviewing complex health assessment and intervention to support children, young people and adults within pathway. External: The post holder will work people in receipt of services their families and carers. They will work alongside people with lived experience to inform and support the delivery of the LDA admission avoidance programme. They will also work with external providers of in-patient services and community provision, NHS England specialist commissioners. A broad range of professionals and members of voluntary services that support the programme, initiating clear navigation across services aiming progress the individual to ensure they are in receipt of the right support at the right time.
