You will play a crucial role in providing holistic care to individuals with diverse mental health needs within the community. Key Responsibilities: Comprehensive Care Management: Conduct thorough holistic assessments of individuals' mental and physical health needs, social circumstances, and support systems. Develop, implement, and evaluate personalized care plans in collaboration with service users and the multidisciplinary team. Coordinate and deliver a range of therapeutic interventions, medication management, and practical support. Monitor and evaluate the effectiveness of care plans, making adjustments as needed. Main duties of the job Arranging, contributing to and documenting care reviews, including telephone, home visits, video and face to face reviews - as appropriate and defined within the care group SOP To supervise junior staff members and to support students in placement To work collaboratively with the multidisciplinary team as well as patients, carers, family members and internal and external agencies. Effective communication skills and self-governance are required. You will be able to demonstrate knowledge of high-level interventions & community assessment skills. Existing qualifications or a willingness to undertake training relevant to the post is expected.
