Job Summary The post holder will primarily see patients face-to-face in a GP surgery. They will provide comprehensive care for presenting patients, including initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. The ideal candidate will have previous experience in a General Practice setting and be proficient in using EMIS Web and associated software integrated within EMIS. Duties and Responsibilities Deliver a high standard of patient care as a prescribing clinician in general practice, utilising advanced autonomous clinical skills and a broad, in-depth theoretical knowledge base. Manage a clinical caseload independently, addressing presenting patients' needs in a primary care setting. Work collaboratively with the multi-disciplinary general practice team to meet patient needs and support the delivery of policy and procedures. Primary Clinical Duties and Areas of Responsibility Serve as the first point of contact for patients presenting with undifferentiated, undiagnosed problems, utilising history taking, physical examination, problem-solving, and clinical decision-making skills to establish diagnoses and management plans. Make professionally autonomous decisions and accept accountability for those decisions. Refer patients to alternative care settings or treat and discharge as appropriate. Instigate necessary diagnostic tests or investigations and interpret findings. Communicate sensitively and effectively to ensure patients are fully informed and consent to treatment. Work with the multi-disciplinary team within the practice and across the broader healthcare community to promote integrated and seamless pathways of care. Contribute to achieving quality targets to maintain high standards of patient care and service delivery. Participate in identifying community health needs and develop patient/family-centered strategies to address them. Help develop and set up new patient services and participate in initiatives to improve existing services. Professional Role Promote evidence-based practice by applying the latest research-based guidelines and contributing to practice-based research. Monitor the effectiveness of clinical practice through audits and peer reviews. Maintain professional registration and participate in continuing professional development opportunities to ensure up-to-date, evidence-based knowledge and competence. Record accurate consultation data in patient records using appropriate coding systems. Stay informed about relevant health-related policies and collaborate with the practice team to implement necessary changes. Demonstrate leadership and identify learning needs for professional growth and improved performance.
