We are currently recruiting for a dedicated and compassionate team committed to providing high-quality social care services to adults in the community. This will be working within an Adults Locality Team playing a crucial role in supporting individuals to live independently and with dignity. We are seeking a Qualified Social Worker to join this Adults Locality Team on a permanent basis. The successful candidate will be responsible for assessing, planning, and implementing care plans for adults with a range of needs. You will work collaboratively with other professionals to ensure the best outcomes for our service users.
Your new role
* Conduct comprehensive assessments of service users’ needs.
* Develop and implement personalised care plans.
* Provide support and advocacy for service users.
* Work in partnership with other agencies and professionals.
* Maintain accurate and up-to-date records.
* Ensure compliance with relevant legislation and policies.
What you'll need to succeed
* Degree in Social Work and registration with Social Work England.
* Proven experience in adult social care.
* Strong assessment and care planning skills.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Commitment to continuous professional development.
* Must be able to drive and have access to a car due to the rural location.
