Job Purpose The role will combine Health and Wellbeing Coach motivational interviewing techniques with the skills of Psychological Wellbeing practices to provide a supportive service, working within Primary Care, supporting patients to take proactive steps to improve their own mental health and wellbeing. They will provide low intensity interventions that will help patients to manage their own recovery. Sessions will be delivered both on a one-to-one and group basis, coaching and motivating through multiple sessions, setting goals and agreeing action planning to support effective outcomes. The role will work closely with other support organisations including Social Prescribers, IAPT services and wider Solihull health and wellbeing services. The post holder will be required to effectively manage a caseload of patients and maintain accurate and concise records and ensure effective onward referral processes are in place and utilised. Duties and Key Responsibilities Conduct patient-centred interviews to identify areas where patients wish to see change and agree action plan and steps in conjunction with patients Make accurate assessments of the risk patients pose to themselves and others Make decisions on the suitability of new referrals, referring clients to alternative services or 'stepping up' their treatment to high-intensity psychological therapy if necessary Support primary care staff to make appropriate referrals to the service, raising awareness of the support available Devise a shared treatment plan with your patient that's delivered via a range of methods such as face-to-face, online or over the telephone Provide low-intensity interventions, using current methods, models and tools to influence positive behaviour change Provide both group and one to one support to patients using a range of mediums including face to face and telephone support Attend multidisciplinary meetings about referrals or clients in treatment Develop strong professional relationships with Primary Care staff, Social Prescribers and external support services Liaise with external agencies including housing, police, local authority, employers and employment support workers where necessary Provide and receive information related to mental health to individuals or groups of clients, relatives, carers, members of the public and professionals Planning and prioritising of own workload to ensure appropriate levels of intervention are provided to each client Completion and maintenance of client records and updating databases to provide key outcome and evidence monitoring of service Signposting and referral to other agencies and services to promote sustained behaviour change and health outcomes Design and completion of relevant paperwork to provide support to clients in the form of assessments, updates and action plans Personal Development Undertake clinical supervision on a regular basis in line with relevant professional guidelines and policies General Report to Line Manager on workload, progress and results Manage own time and resources Prioritise workload in liaison with Line Manager Any other duties as agreed with Line Manager, commensurate to the post Health and Safety Take reasonable care for the Health and Safety of yourself or other persons who may be affected by your act or omissions Co-operate with the Company to comply with any Company safety rule and/or any duty or requirement imposed under legislation or relevant Codes of Practice Maintain safe and clean conditions in your working area according to the Companys housekeeping standards Work in accordance with information and training provided Equal Opportunities It is the responsibility of each employee to abide by the Companys Equal Opportunities Policy Data Protection At all times during employment, to comply with the provisions of the Data Protection Act 1998 and with any policy introduced by the Company to comply with the Act including GDPR legislation.
