1. Select and apply OT assessments underpinned by MOHO Theory, for a designated caseload to support case formulation and intervention planning. 2. To work with service users to identify OT goals as part of the overall care plan, working within the CPA process. 3. To plan and implement individual and/or group interventions, in collaboration with the service user, using graded activity to achieve therapeutic goals. To identify any significant risk factors in line with local policies, with support from senior staff 5. To monitor, evaluate and modify treatment for service users with multiple needs in order to measure progress and ensure effectiveness of intervention. 6. To consider both physical and mental health needs as part of the occupational therapy process 7. Assess the occupational and vocational needs of the service user group signposting and/ or supporting access to community and employment opportunities where appropriate 8. Provide clinical training and supervision to service users extended support network as appropriate. Provide information, advice, and assessments to the team with regard to the suitability of placement
