Job summary Job overview The Care and Support Worker will work within a day service setting supporting people who have a wide range of learning, physical and health needs and also includes people who need support when they become distressed. You will participate in and access activities in the centre and the community and work as part of a multi-disciplinary team working with people who attend Hollacombe, their parents, carers and other professionals'. You will provide support in accordance with individuals care plans and carry out tasks allocated by the senior team. You will act as an escort on a minibus with up to 8 other passengers with a tail lift and have responsibility alongside the driver for the safe transportation of people to and from the day service.
