Job Title: Adult Social Worker – Discharge to Assess Team

About the Role:
We are looking for an enthusiastic and dedicated Adult Social Worker to join the Discharge to Assess (D2A) Team. This role is essential in ensuring that adults leaving hospital receive timely assessments, support, and care planning to facilitate a safe and effective transition back into the community.
As part of the D2A team, you will work closely with hospital staff, community health teams, care providers, and families to ensure individuals receive the appropriate level of care, promoting independence and well-being while avoiding unnecessary hospital admissions or readmissions.
Key Responsibilities:
* Undertake Care Act 2014 assessments for individuals transitioning from hospital to home or an alternative care setting.
* Develop strengths-based, person-centred care plans, ensuring individuals receive appropriate support to regain independence.
* Work with health professionals, occupational therapists, and community services to coordinate effective discharge planning.
* Identify ongoing care and support needs, ensuring timely referrals to long-term care services where necessary.
* Conduct risk assessments to ensure individuals are discharged safely, with appropriate safeguarding measures in place.
* Implement safeguarding procedures where necessary, working closely with adult safeguarding teams.
* Monitor and review support plans, adjusting services as needed to meet changing needs.
* Collaborate with NHS teams, housing services, voluntary organisations, and care providers to ensure seamless service delivery.
* Attend multi-disciplinary meetings, advocating for the needs and preferences of service users.
* Ensure compliance with statutory requirements, local policies, and national best practices.
* Maintain accurate and up-to-date case records, ensuring all assessments and care plans comply with the Care Act 2014.
* Work in accordance with Mental Capacity Act 2005, Continuing Healthcare (CHC) processes, and local authority policies.
* Ensure all interventions align with strengths-based and person-centred social work practice.
Qualifications and Experience Required:
* Degree in Social Work (or equivalent) from a UK university
* Registration with Social Work England
* Post-qualifying experience in adult social care, hospital discharge, or community-based care planning.
* Strong knowledge of Care Act 2014, Mental Capacity Act 2005, and NHS Continuing Healthcare (CHC) frameworks.
* Ability to manage complex casework and risk assessments.
* Strong problem-solving and decision-making skills.
* Experience working in a multi-agency environment, ensuring a coordinated approach to care.
