Position
The role of the Team Coordinator for South Tyneside is an important one within the community team. You will represent Coquet Trust to the people we support, their families, and their wider circles of support, as well as external agencies. As a leader, you will ensure that your team delivers person-centered care, empowering individuals to lead fulfilling, independent lives.
Your role will be key to supporting incredible, unique individuals, helping them thrive while leading and inspiring a dedicated team of Support Workers.
Support Responsibilities:
* Support individuals in accordance with their person-centered plans, ensuring their needs, goals, and preferences are at the heart of everything you and your team do;
* Empower individuals to become active members of their community, fostering independence and life skills;
* Provide hands-on support for daily living activities such as shopping, meal preparation, maintaining their home, and budgeting;
* Lead your team to promote positive behaviour support and emotional regulation techniques;
* Safeguard vulnerable individuals by following Coquet Trust’s Safeguarding procedures;
* Assist individuals in accessing volunteering or employment opportunities where appropriate.
Senior Responsibilities:
* Act as the first point of contact in the absence of the Support Manager;
* Lead and inspire your team through effective supervision, clear communication, and role modelling best practices;
* Oversee the creation of effective staff rotas, ensuring the team meets the needs of the individuals supported;
* Support the well-being, safety, and development of individuals, ensuring compliance with Coquet Trust’s policies and safeguarding protocols;
* Supervise, train, and motivate Support Workers, ensuring high standards of care are delivered;
* Maintain accurate documentation, including support plans, financial records, and health and safety audits.
For This Role You Will Support the Following Individuals:
A Vibrant and Creative Young Woman
You will work with a lively and sociable young woman with a moderate learning disability, ADHD, and epilepsy. She thrives on structure, creative activities, and social interaction. She loves exploring arts and crafts and enjoys an engaging, supportive environment. Occasionally, she can experience behaviours of concern when routines are disrupted or situations become overstimulating.
You will guide and support your team to provide a patient, consistent approach, promoting her independence, emotional regulation, and participation in activities she enjoys, ensuring a stable and positive environment.
A Kind-Hearted, Thoughtful Individual
You will oversee the team supporting a young man with autism and paranoid schizophrenia who loves music, meditation, and gaming. He flourishes in calm, structured environments but can experience anxiety and sensory overload.
Your leadership will ensure his team offers reassurance and a sense of security while helping him manage his mental health, engage with his community, and explore meaningful activities that enhance his well-being.
A Determined Young Woman Who Loves Music and Family
This role involves supporting a young woman with a mild learning disability and autism. She is passionate about music, karaoke, and cooking and values her close family connections. Building trust is vital for her.
You will ensure her support team helps her develop emotional regulation skills, build healthy routines, and explore her independence, while also encouraging her to engage with activities that nurture her passions and strengthen her confidence.
A Resilient and Creative Young Woman
This young woman has moderate learning disabilities and mental health needs. She is incredibly resilient and enjoys routines, visual plans, and social connections. Her creative spirit shines through in her structured daily activities.
As Team Coordinator, you’ll lead your team to empower her to build independence, manage her emotional well-being, and enjoy structured activities like meal planning, community outings, and creative pursuits.
A Driven and Ambitious Young Adult
You will oversee the support team for a young adult with a mild learning disability, ADHD, and a personality disorder. She has ambitious dreams of living independently and developing meaningful relationships.
You’ll lead a team that helps her achieve her goals by focusing on emotional regulation, building routines, and promoting her personal safety and independence, all while celebrating her determination and aspirations.
Requirements
Professional Qualities:
* Experience supporting individuals with learning disabilities, autism, or mental health needs;
* A commitment to person-centered care that empowers individuals to achieve their goals;
* To represent Coquet Trust in a professional manner and maintain the good reputation of the organisation at all times;
* Be committed to the Coquet Trust’s values in everything that you do;
* Have experience of leading, developing and motivating team members in order to deliver a quality service to the people that we support;
* To be flexible in your approach to work and be prepared to work at different locations or wherever it is deemed that you are able to contribute most effectively to the overall service;
* Ability to work both independently and as part of a team;
* Clear written and verbal communication style;
* IT skills and numerical ability;
* Willing to undertake required training, sometimes off site and outside normal working hours;
* Willing and able to travel between locations as required;
* Available for flexible working patterns including evenings, weekends, sleep-ins and nights as required;
* Experience of communicating with professionals or other agencies;
* NVQ Level 3 or above in social care or equivalent (or willingness to obtain it within an agreed timeframe).
