We are seeking a skilled and compassionate Band 7 Complex Care Case Manager to join our Complex Care and Case Management team. In this pivotal role, you will focus on preventing admissions under the Mental Health Act and expediting safe discharges. You will work closely with multidisciplinary teams (MDT) to ensure personalised support is in place for effective discharge planning, chair Care (Education) and Treatment Reviews (C(e)TRs), and complete Learning Disabilities Mortality Review (LeDeR) processes.

This post is aligned to Newcastle/Gateshead; however, the post holder would be expected to work across the region if required.

Main duties of the job
Key Responsibilities:
* Preventing Admissions: Work proactively to prevent unnecessary admissions under the Mental Health Act.
* Safe Discharges: Expedite safe discharges from inpatient settings by ensuring all necessary support and resources are in place for service users.
* Multidisciplinary Collaboration: Collaborate with MDTs, including healthcare professionals, social services, and other relevant agencies, to develop, implement and review personalised packages of care and support.
* Chairing C(e)TRs: Chair Care (Education) and Treatment Reviews to ensure that care plans are comprehensive, person-centred, and effectively address the needs of individuals.
* LeDeR Reviews: Conduct Learning Disabilities Mortality Reviews (LeDeR) to identify areas of improvement and contribute to the enhancement of care quality and safety.
* Documentation: Maintain accurate and up-to-date records of all interactions, assessments, and care plans in compliance with NHS standards and regulations.
* Training and Support: Provide guidance, training, and support to junior staff and colleagues to promote best practices in complex care management.
* Quality Improvement: Participate in quality improvement initiatives and contribute to the development of service policies and procedures.

* Qualifications: Registered Nurse (RN) with current registration.
* Experience: Significant experience in managing complex cases within the NHS or Local Authority, with a strong understanding of mental health, learning disabilities, and autism.
* Skills: Excellent assessment, planning, and organisational skills, with the ability to manage a diverse caseload effectively.
* Communication: Strong interpersonal and communication skills, with the ability to build positive relationships with service users, families, and multidisciplinary teams.

Person specification
Knowledge, Training and Experience
* Professional Registration
* Educated to Masters Level equivalent experience
* Evidence of ongoing professional development
* Degree or equivalent
* Advanced knowledge of physical health conditions that are associated with individuals with Learning disabilities, including management of such conditions
* Facilitation of Multi-Disciplinary Team meetings and the ability to lead and influence change
* Relevant experience of working in a complex organisation preferably the NHS/LA
* Experience of reviewing the health and/or nursing or social care needs of individuals
* Multi-agency safeguarding processes
* A background that demonstrates inter-agency and multi-agency working
* Experience of writing comprehensive reports and delivering high quality work to tight deadlines
* Experience of analysing and interpreting complex information, including legislation guidelines and statistical
* Use of standard computer programmes and data base management
* In depth knowledge of the Mental Capacity Act 2005 and Mental Health Act 1983/2007 and how this relates to practitioners and individuals
* Understanding of the legislative framework supporting the framework
* Knowledge and understanding of safeguarding processes and recognition of risk/vulnerabilities
