Specialist Community Dietitian (0.6WTE of role):
Act as a specialist for the local community managing the dietetic service provision to community Hospitals in-patients and staff and to community based patients in need of dietetic advice and review. Take responsibility for the provision of dietetic services, including appropriate dietary counselling, lifestyle advice and nutrition education to clients, carers, families and health care professionals in their own homes, Nursing Homes, Residential Homes and Locality hubs within NW Surrey. Support the ongoing delivery of dietetic educational programmes on nutritional care for care home staff, GPs, and other health care professionals working across NWS. Lead on development of dietary protocols and guidelines for the service and contribute to the policies for the team as a whole.
Take an active role within the general functioning of the Dietetic Department, including contributing to student training and supporting junior team members as required.
Specialist Primary Care Network Dietitian (0.4WTE of role):
Delivery of the PCN dietetic service will include initial clinical assessment, diagnosis, evaluation and treatment of patients' nutritional care with a holistic view, referring onto other agencies when required. The PCN Dietitian will: be responsible for leading the provision of dietetic services to a PCN, deliver dietetic clinics in the PCN, and support integrated and collaborative provision of dietetics services alongside community and acute dietetic services in the locality. Be responsible for monitoring the effectiveness of the PCN dietetic service and work to sustain and improve this. Proactively seek engagement from local PCN leads to identify areas for service improvement, aiming to develop pathways of improved dietetic care to meet the health needs of the local population and population priorities in line with the NWS Alliance and Surrey Heartlands ICB.
