Rehabilitation Support Worker - Band 3

30 Hours per week - Permanent
Could you support patients when they returnhome from hospital? Are you keen to help prevent people being admitted tohospital? Would you like to be a valued member of our forward-thinkingcommunity teams? If so, then come and join our Home First programme.
We have a Band 3 Rehab SupportWorkers/Therapy Assistants trained to NVQ III standard (or equivalent) orpeople willing to train to this level. We want to meet applicants keen todevelop into this rehabilitation role or who have existing transferable skills.
Youll be part of the Amesbury CommunityTeam, Amesbury Health Centre and you will be working across a wider area in ourSouth locality. You will be supported by our physiotherapists, occupationaltherapists and nurses and will work alongside our Assistant Practitioners andexisting Support Workers. You will play a vital role helping people torehabilitate and regain their independence in their own home.
Main duties of the job
If you have experience in front line NHScommunity patient care then thats great but its not essential. We wantenthusiastic, motivated and hard-working people who can work flexibly andefficiently. We would like you to have effective communication skills, good ITknowledge, and the enthusiasm to develop.
Wiltshire Health and Care is developingtraining specific for these roles and will provide you with a supportive,friendly and happy environment in which to learn new skills.
The community teams deliver services from 7amto 10pm, seven days a week. We can talk about your preferred hours of work andworking patterns at the interview, or you can call the following manager for achat.
For this post you will need to hold a UKDriving Licence and have access to a car.
