Were seeking two skilled Complex Needs Practitioners to join our team in Aylesbury. About the role: Deliver Person-Centred Interventions : Engage individuals through a variety of tailored interventions, empowering them to reduce risks, build resilience, and achieve meaningful, positive outcomes. Flexible, Community-Based Work : Work flexibly across community settings and within our multi-agency partnerships. Your caseload will include referrals from both community and multi-agency sources, including health and social care sectors. Address Holistic Wellbeing Needs : Support clients in all aspects of their wellbeing, addressing a range of complex issues, such as knife crime prevention, 5-step family interventions, exploitation, physical and emotional health, and substance misuse. Requirements: Relevant Qualifications : Hold a qualification in health or social care, youth and community work (e.g., NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies). We also welcome candidates with relevant experience in the substance misuse field who are committed to completing NVQ Level 3 in Health and Social Care. Self-Motivated and Confident : Bring a proactive, energetic approach, with the confidence to take initiative and make a difference. Experience in Multi-Agency Collaboration : Demonstrated experience working effectively with both voluntary and statutory agencies, engaging with health professionals, and building rapport with clients across diverse settings. Adaptable and Detail-Oriented : A flexible approach to work, with strong record-keeping and report-writing skills. Willingness to work across the community and within multi-agency partnership settings, providing outreach support with a central hub base at our head office.
