Who you’ll be supporting & more about the role:
As a Complex Care Assessment Practitioner, you will lead the assessment and development of personalized care plans for individuals with learning disabilities and mental health issues. Your role will involve:
* Person-Centered Assessments: Conducting thorough assessments to identify individual needs and outcomes.
* Collaboration: Working closely with clinical teams and external professionals to ensure comprehensive care.
* Quality Assurance: Leading quality evaluations and ensuring compliance with regulatory standards.
* Advocacy: Being a strong advocate for the individuals we support, ensuring their voices are heard.

Essential Criteria
To be successful in this role, you will need:
* Education: Degree in health and social care (or equivalent) and further postgraduate training in relevant clinical or research areas.
* Experience: Proven experience in care and support assessments, particularly with individuals with learning disabilities and/or mental health issues.
* Knowledge: Understanding of Positive Behaviour Support, Transforming Care Partnerships, and relevant regulatory environments.
* Skills: High-level communication skills (written and oral), strong IT skills, and the ability to manage complex assessments.
* Competencies: Ability to maintain focus during assessments, manage emotionally stressful situations, and build strong relationships with stakeholders.
* Qualifications: Trusted Assessor qualification (England only) and experience working with Local Authorities and public sector organizations.
