We have an exciting opportunity in our Osteoporosis and Fracture Liaison Service for a nurse specialist to provide specialist care & support to patients with Osteoporosis and those at risk of fragility fractures.
You will ensure patients are supported within the specialist field and will work closely within the multi-disciplinary team comprising of consultants, nurse specialists, radiographers and our secretarial team. We receive referrals from across the Trust, along with GP referrals. We have a close relationship with the Trauma and Orthopaedic Team and DME.
We are looking for someone who has experience in either of the above specialities who is ready for a new challenge, with at least 3 years post registration experience. Clinical Decision Making Module and Non Medical Prescribing are desirable.

Main duties of the job
* To support patients by maintaining and developing a quality service, alongside the Osteoporosis and Fracture Liaison CNS team and the wider MDT.
* To support the team with a caseload of patients requiring specialist Osteoporosis nursing support.
* To support the team by taking on clinics and providing support for patients throughout the pathway.
* To support patients and staff on the ward to help care for those who have sustained a fragility fracture.
* Ensure the delivery of agreed professional standards of care within the specialist area.
* To support and participate in research and audit within specialist area.
You will work across the Royal Derby Hospital and Florence Nightingale Community Hospital, providing both face to face and telephone clinics in an outpatient setting. Inpatient work is also part of the role; however, this is worked within normal clinic hours. Community education sessions are provided to patients 2-monthly so you will be expected to support these sessions on a rota basis throughout the year. You will report directly to the Lead CNS in the Osteoporosis Service.
Working hours are normally 8.30-4.30. Working days to be discussed at interview.

Education, Training and Qualifications
* NMC registration & revalidation requirements.
* Demonstrable post registration experience at B5 level in Trauma & Orthopaedics, Care of the Elderly or other specialist services.
* IT literate.
* Evidence of degree / working towards higher degree or equivalent experience.
Experience & Knowledge
* Able to assess associated risks and take appropriate action.
* Manages the clinical workload, prioritising and re-prioritising.
* Experience at B6 level Clinical Decision Making Module Non-Medical Prescribing.
* Experience of working in an outpatient setting.
Communications and interpersonal skills
* Communicates condition related, sensitive information to patients, public and staff.
* Accurately maintains patient/staff records, records own data.
* Prioritises problems, assesses associated risks and takes appropriate action.
