The role of the Team Leader is to provide high quality care and support, as defined in the care and support plan, to our clients within the clients' home, therapeutic environment or in the community. The Team Leader is responsible for managing the support team to ensure day to day care and strategic outcomes are met.
Main duties of the job
The Team Leader is expected to provide leadership and support for care staff, act as a role model for good clinical and care governance practice, uphold the rights, choice and dignity of Service Users, and ensure Libertatem Healthcare Group policies and procedures are followed at all times. They are also responsible for managing the team, including rota management, supervision, feedback, and ongoing training needs, as well as delivering a high-quality care service without discrimination.

2. Provide a weekly update via email to Clinical lead & coordinator.
3. Act as a role model for good clinical and care governance practice
4. Uphold the rights, choice and dignity of Service Users.
5. Ensure Libertatem Healthcare Group policies and procedures are followed at all times.
6. Ensure Care Quality Commission standards are met for all clients.
7. Deliver a high quality care service without discrimination
8. Notify clinical lead or coordinator of any training requirements
9. To liaise with clinical lead and take lead on delivery of MDT recommendations Team management/ Leadership
10. Provide leadership to the team acting as a positive role model at all times.
11. Motivate, support and mentor the staff when needed.
12. Rota management/ annual leave
13. Monthly supervision, feedback and ongoing training needs
14. Team meetings/ minutes and dissemination of information and follow up directed steps
15. Able to have a proactive, positive approach Behaviour and strategy management
16. Updating of care plan, risk assessments and behaviour strategy plan with incorporation of professionals information
17. Monthly meetings with parents
18. Join MDTs, feedback these minutes to the team
19. Ensure meds stock been ordered
20. Bi-weekly discussion with AP to check in and give her the chance to raise concerns
21. Promoting a full and active live full of opportunities and choice
22. Someone who will have the difficult discussions with the team
23. Outcome lead goal planning in line with activity schedule
24. Monthly based Audit of Community based activities
25. All instructions given by therapists are embedded in AP life and updated accordingly.
26. Clinical lead will discuss tasks with Team Leader to rotate tasks.
27. Audit of behaviour management used – restraints, de-escalation techniques etc.
SupportingIndependenceandcontinuedtherapies
28. Promote self-care and self-management to achieve a maximum level of independence for service user. This may include: - Provision of support to re-learn or find new ways of maintaining independence. - Assistance with Service Users’ mobility, feeding, washing, bathing and dressing. - Assistance with domestic care tasks such as laundry, ironing, and household cleaning. - Preparation of food and drinks, kitchen cleaning and hygiene. - Assisting Service Users to attend appointments, go shopping, collect pensions etc.
29. Provision of emotional support, and support to increasing Service Users’ confidence and motivation.
30. Assist with the use of existing equipment to promote independence, and provision of access to specialist equipment or technology as appropriate.
31. Administer medication as designated in the care and support plan in accordance with Company policies and procedures.
32. Assist Service Users with therapeutic activities as directed by the care and support plan and as directed by members of the multidisciplinary team.
33. Deliver support in a manner, which is sensitive to the service user’s culture, disability, race, age, sexuality, marital or civil partnership status, transgender status or religion/belief.
34. Document clearly in the daily records on the progress of rehabilitation programs i.e. observing, recording, and reporting on changes in a Service User’s physical and/or emotional condition.
Communication
35. Assist the healthcare coordinator and clinical lead nurse by communicating all information to ensure Service Users’ needs are met.
36. Work in partnership with all members of Libertatem team and report any changes in a Service User’s mental or physical health, or social conditions
37. Ensure that concerns around safeguarding issues or equality and diversity are reported in line with Company procedures.
38. Report any serious incidents, accidents or near misses to the office team, following company procedure.
39. Report any hazards or potential hazards that impact on the safe delivery of care to the office team, in line with Company procedure.
40. Report to the office team professional any concerns, complaints, or suggestions for improvements raised by Service Users and/or their Carers.
41. Report to the office team promptly and appropriately to protect service users from neglect or abuse.
42. Maintain clear, concise and legible Service User records, according to Company policy and procedure.
43. Use Libertatem Healthcare’s ‘On Call’ system for additional support if required, in accordance with Company guidance.
44. Ensure that all computer security procedures are followed.
45. Always act in a manner which respects the confidentiality of Service Users, staff and any other involved parties.

* The Team Leader will be required to complete the mandatory and client-specific training, both in-house and external, to achieve the specified level of competency for each client with whom they work.
