The role of the Team Leader is to provide high quality care and support, as defined in the care and support plan, to our clients within the clients' home, therapeutic environment or in the community. The Team Leader is responsible for managing the support team to ensure day to day care and strategic outcomes are met.
Main duties of the job
The Team Leader is expected to provide leadership and support for care staff, act as a role model for good clinical and care governance practice, uphold the rights, choice and dignity of Service Users, and ensure Libertatem Healthcare Group policies and procedures are followed at all times. They are also responsible for managing the team, including rota management, supervision, feedback, and ongoing training needs, as well as delivering a high-quality care service without discrimination.

Job responsibilities

Team Leaders are expected to:
* Provide leadership and support for care staff
* Provide a weekly update via email to Clinical lead & coordinator.
* Act as a role model for good clinical and care governance practice
* Uphold the rights, choice and dignity of Service Users.
* Ensure Libertatem Healthcare Group policies and procedures are followed at all times.
* Ensure Care Quality Commission standards are met for all clients.
* Deliver a high-quality care service without discrimination
* Notify clinical lead or coordinator of any training requirements
* Liaise with clinical lead and take lead on delivery of MDT recommendations
* Provide leadership to the team acting as a positive role model at all times.
* Motivate, support and mentor the staff when needed.
* Manage rota and annual leave
* Conduct monthly supervision, feedback, and ongoing training needs
* Facilitate team meetings, document minutes, and disseminate information
* Maintain a proactive, positive approach
* Update care plans, risk assessments, and behaviour strategy plans
* Conduct monthly meetings with parents
* Join MDTs and provide feedback to the team
* Ensure medication stock is ordered
* Conduct bi-weekly discussions with AP to address concerns
* Promote a full and active life full of opportunities and choice
* Lead difficult discussions with the team
* Plan outcome-led goals in line with the activity schedule
* Conduct monthly audits of community-based activities
* Ensure instructions from therapists are integrated into AP life
* Rotate tasks with the clinical lead
* Audit behaviour management techniques used
Supporting Independence and Continued Therapies
* Promote self-care and self-management to achieve maximum independence for service users.
* Assist with mobility, feeding, washing, bathing, and dressing.
* Help with domestic tasks such as laundry, ironing, and cleaning.
* Prepare food and drinks, and maintain kitchen hygiene.
* Support service users in attending appointments and other activities.
* Provide emotional support to enhance confidence and motivation.
* Assist with the use of equipment to promote independence.
* Administer medication as per care and support plan.
* Assist with therapeutic activities as directed.
* Deliver support sensitive to the service user’s diverse needs.
* Document progress in rehabilitation programs accurately.
Communication
* Communicate effectively with the healthcare coordinator and clinical lead.
* Collaborate with the Libertatem team to address changes in service users’ health.
* Report safeguarding concerns in line with company procedures.
* Report incidents or hazards that affect care delivery.
* Maintain clear and concise service user records.
* Utilize the 'On Call' system for additional support as needed.
* Respect confidentiality of service users and staff.

Data Protection and Confidentiality
The Support Worker must respect confidentiality in line with the Data Protection Act, ensuring fair and lawful processing of information.
Training
The Team Leader will complete mandatory and client-specific training to achieve competency for each client.
Participate in regular supervision and appraisal meetings.
Demonstrate commitment to personal development and suggest additional training for the team.
