Job Description
Key Responsibilities
* Conduct comprehensive assessments of patients to identify their occupational needs and goals.
* Develop and implement personalised care plans in collaboration with patients and their families.
* Provide therapeutic interventions that enable patients to engage in meaningful activities.
* Educate patients, families, and carers on strategies to manage daily living activities and enhance independence.
* Work closely with other healthcare professionals within the Integrated Care Team to ensure coordinated and holistic care delivery.
* Monitor and evaluate the effectiveness of interventions and make adjustments as necessary.
* Maintain accurate and timely documentation in accordance with departmental policies and standards.
* Participate in team meetings, case discussions, and contribute to service development initiatives.
* Provide supervision and support to junior staff and students as required.
Qualifications and Experience
* Degree in Occupational Therapy and current registration with HCPC.
* Significant experience in a clinical setting, ideally within community or integrated care.
* Strong assessment and intervention skills, with the ability to work autonomously.
* Excellent communication and interpersonal skills.
* Ability to work collaboratively within a multidisciplinary team.
* Knowledge of relevant legislation and best practices in occupational therapy.
