Job Title – Therapy – Physical Therapist (PT)
Location – Kilmarnock, VA
Pay – $2126 – $2246/wk
Schedule Details – Day, 5X8

Position Details
Start 12/30/24 or 1/6/25
Travel candidates only (Must be 50 miles outside of facility)
Dysrhythmia NOT exam needed
Open to temp to perm
Experience preferred, but will consider new grads 8-hour DAY shifts M-F (likely able to pick up a weekend shift at the hospital if wanting a weekday off)
BLS and VA license required
Unit Specific Details
Outpatient facility ( 43 William B. Graham Court Kilmarnock, VA) attached to hospital
Cyrotherapy, Heating pads, Antigravity devices, TENS units, Parallel Bars – Equipment consistent with outpatient setting
Experienced staff including: Full time PT, Supervisor, OT, Speech therapist and Rehab tech onsite Patient Pop: Geriatric Ambulatory Outpatient Orthopedics and Neuro mainly including joint replacements, post stroke, balance, etc.
Summary:
Provides skilled therapeutic interventions to patients of CMHP through the use of their educational skill. This may
involve outpatients, inpatients, pediatrics and in off-site locations. Services the patients of CMHP as a part of the
entire continuum of professional care that CMHP delivers.
Duties & Responsibilities:
Evaluates, plans, and implements Physical Therapy services for patient per standards of practice of the profession, laws
governing practice by the State of Ohio, and facility and department policies and procedures.
Collaborates with the patient, family, and treatment team to maximize functional outcomes for the patient.
Obtains and interprets prescriptions/orders for services with physician/referral source as required.
Assesses and evaluates patients using appropriate non-standardized and standardized tests.
Ensures completion of patient pre-certification/authorization requirements as needed.
Demonstrates knowledge in growth and development necessary for evaluation of adolescent, adult, and geriatric patients (pediatric as required by site).
Develops individualized treatment program consistent with patients age, diagnosis, functional status, and personal
goals.
Ensure the patient and/or family are included in development of attainable goals.
Selects and administers appropriate techniques, modalities, and interventions to maximize outcomes.
Monitors patients response to treatment and modifies the plan as needed.
Demonstrates knowledge in growth and development necessary for treatment of adolescent, adult, and geriatric patients. (pediatric as required by site) Locations for treatment and evaluation include the hospital/clinic and settings in the
community.
Effectively communicates to patient, family/caregiver and treatment team both orally and in writing.
Collaborates with the patient, family/caregiver and treatment team in planning for discharge. Provides patient and
family/caregiver education and training.
Makes appropriate discharge recommendations and obtains and/or recommends appropriate assistive equipment.
Promotes the profession by participating in the Clinical Education program and providing education to others.
Acts as a fitness/wellness instructor as needed for community programs.
Maintains competency by participation in continuing education, in-services and other learning opportunities.
Meets standards for all required reviews of competency.
Shares responsibility for common goals of the team by carrying caseload consistent with peers, maintaining the work
environment and inventories, adjusting individual responsibilities to meet needs of department/work environment and
positively representing department and profession to others.
Participates in process improvement by identification of improvement opportunities for patient care, data collection and
solutions to identified problems.
Education & Requirements:
Minimum Education: 4yr Bachelors Degree Physical Therapy
Graduate of accredited program in Physical Therapy with minimum of Bachelors degree.
LICENSURE/CERTIFICATIONS (must be non-expired/active unless otherwise stated):
Licensed Physical Therapist in the State of Ohio; CPR
Holds a current Physical Therapy license from the State of Ohio in good standing
Maintains CPR Certification.
Graduate of accredited program in Physical Therapy with Doctorate level degree preferred
Certification in a specialty area, for example: OCS, SCS, NDT, Lymphedema, Womens Health, etc. preferred
