Job summary This is an exciting time to join the community service as we continue to grow and work as a seamless multidisciplinary team, so we can be there to support more people at end of life. Our services work across 7 days a week delivering a mixture of direct planned and responsive care and supporting other professionals through our expertise and experience. Above everything else, our service is patient centred. The role is predominantly office based and will provide comprehensive administrative and clinical support to the CPCT Hub 5 days a week. It may also include deployment to support outpatient clinics, and urgent community visits when required, and provide ongoing support for the stable patient caseload. Main duties of the job The post holder will work without direct supervision to facilitate and coordinate daily patient flow within the Community Palliative Care Team (CPCT) Hub. Utilising their own initiative, the postholder will proactively identify opportunities and actions to support the clinical team in the Hub, patients, and families.
