Job summary
Heart & Lung Transplant and Ventricular Assist Device Coordinator
The post holder will work in conjunction with members of the multidisciplinary Heart and Lung Transplant/VAD (left ventricular assist device) team to coordinate the nursing and managerial aspects of the patient pathway, pre and post heart and lung transplantation and mechanical circulatory support insertion. The role is a trainee Band 6 position which will develop to a Band 7 nurse specialist on achieving all areas of competencies.
Main duties, tasks & skills required
The role includes:
* Provide expert information, education, support and advice to patients, families, colleagues and other health professionals in the inpatient, outpatient or community settings within a 24 hours service.
* Coordinate the whole patient pathway for patients pre and post heart and lung transplantation or mechanical circulatory support, including admission to and discharge from hospital for assessment, procedures and post-operative care.
* Help develop and participate in nurse led clinics.
* Receive and act upon all National and European cardiothoracic organ offers under the supervision of Mentor and Consultant Surgeon.
* Organise all aspects of cardiothoracic retrieval within the area of responsibility.
* Participate in a 24 hour on-call rota.
* Attend and participate in Internal and external meetings.
* Educate at a local and national level.

Qualifications
Essential
* Registered Adult nurse on NMC register
* Evidence of post registration CPD / Post registration qualification in appropriate area of speciality or professional knowledge supplemented by specialist experience and short courses to degree / masters level equivalent FHEQ Level 6 /7
* Recognised mentorship course
Desirable
* To have or working towards a MSc/ Masters qualification
* Advanced health assessment module -To have or working towards or willingness to undertake
* Non-medical prescriber -To have or working towards or a willingness to undertake
Experience
Essential
* Substantial experience in taking charge of an acute ward or area
* Minimum 2 years cardiology / respiratory experience, awareness and knowledge
* Experience at working with acute and chronic diseases
* Experience of working autonomously
* Able to demonstrate commitment to high quality patient care
* Experience and enthusiasm for developing educational programmes and teaching / assessing and supervising others in a clinical setting
* Awareness of current relevant NHS policy context relating to the speciality
* Clear understanding of the nurse specialist role
* Evidence of implementing research based practice
* Must demonstrate a willingness to teach at a local and national level
* Evidence of supporting others in a clinical environment
