An exciting opportunity has arisen to join our Acute Clinical Team in Bridgend as an Advanced Clinical Practitioner/Trainee Advanced Clinical Practitioner.
ACT is part of the wider Community Resource Team (CRT) in Bridgend and provides a hospital admission avoidance service, responding rapidly to people in crisis because of frailty and/or managing acute medical problems at home.

Since merging into the new Cwm Taf Morgannwg University Health Board, the Bridgend ACT now has an updated vision for this service.
Come join us on this exciting journey to develop and grow this intermediate care service that aims to keep people home and avoid admissions to hospital.
Professionals who are Nurses, Physiotherapists and Paramedics by professional registration are invited to apply for this position.
While Professionals who meet the person specification and hold appropriate M.Sc. level education (such as an M.Sc. in Advanced Clinical Practitioner and Independent Prescribing qualifications) are desirable, we will consider applicants who can demonstrate they are a good fit for the service and have relevant, transferrable skills, education and experience to be taken on in a trainee ACP role.
Main duties of the job
