Job summary The Lower GI Cancer Clinical Nurse Specialist is responsible for the provisions of the coordination of patient care for the Central Liverpool site, which includes providing expert clinical leadership, specialist advice to manage the care of patients with colorectal cancer. They will act as a role model demonstrating high standards of care and providing clinical leadership to others. They will develop patient pathways and be responsible for developing the nursing service, while working with the multidisciplinary team to ensure that services complies with national standards. The post holder will work autonomously and develop collaborative relationships with others. They will offer advice, support and education to clinical teams across the trust and cancer network. The role is key in offering patients and families the skills of a clinical nurse specialist in symptom control, emotional and spiritual care. Deliver evidence based individualised patient care according to patient's changing health care needs, through holistic needs assessment (HNA). To provide expert knowledge and advice in own specialist area, ensuring maintenance of clinical excellence underpinned by evidence-based practice. Main duties of the job Develop new and innovative ways of working within CCC and the wider network area. Develop newpatient pathways and processes that enable consultant teams to work more effectively andefficiently; thereby contributing to a reduced patient wait in clinics and improved follow upprocesses.Support standards of safe practice within the Trust that ensures the appropriate person, in theappropriate place, in an efficient and timely manner, cares for patients identified. Co-ordinate directadmission of patients presenting with complications of treatment or needing supportive therapy,through agreed protocols and processes.Contribute to the Multi-Disciplinary Team discussion, participate in the advanced physical andpsychosocial assessment, interpreting clinical and diagnostic findings in order to determine adiagnosis, evaluate interventions/treatments for patients with complex needs and develop anevidence based plan and co-ordination of care.
