Job summary Stroke specialist nurse Band 6 An exciting opportunity has arisen for a Full time permanent Band 6 Stoke specialist nurse to join the Stroke Team at Ysbyty Gwynedd Hospital. We are looking to recruit enthusiastic and motivated Band 6 Registered Nurse with an interest in Stroke and be passionate about delivering high standards of care. We are looking to welcome an experienced registered nurse to join our small team of Stroke Coordinators, to develop and bring new ideas to the developing service, in order to provide exceptional nursing care. Main duties of the job The role is centred at providing excellent patient centred care through the patient's journey through from admission to discharge into primary care, identifying and monitoring the long-term needs of stroke patients through close liaison with the primary care health teams. In addition, the successful candidate will be working alongside the Multidisciplinary team on Prysor Ward, which is designed to accommodate Strokes from admission to discharge or transfer to the rehabilitation area once deemed medically fit. ASU houses an acute level 2 admission area that facilitates the holistic care of stroke patients post cerebral thrombolytic therapy. ASU provides a complete Multi-disciplinary team approach with all relevant therapies being based within the ward template and this allows smooth transition of care and collaborative working at its very best. In conjunction with the Senior Stroke Coordinator, ensure that the requirements of the Sentinel Stroke National Audit Programme are met. Maintain an environment of continual improvement in which clinical excellence can flourish to enhance the patient experience and improve clinical outcomes. The post holder will act as a signposting resource to other healthcare professionals requiring specialist advice.
