The Post holder is a central element to the effective and efficient management of the Trust Risk & Incident Management System (InPhase). This role requires that accurate information is available to enable the Trust to extract accurate statistical data. The Post holder will work within departmental guidelines and under the direction of the InPhase Project Manager; using own initiative to support the delivery of effective Risk and Incident processes, in particular, incident reporting. We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients’ homes. We also are incredibly proud to have almost 150 volunteers here at CHFT. We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children’s and young people’s services; end of life care and outpatient and diagnostic imaging services. We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. Administration of Incidents Checking and reviewing data for accuracy and completeness and refer incidents that require follow-up to senior members of the team. To support the safe and timely management of all incidents. Checks to be carried out against an established criteria/checklist to ensure consistency of recording. To liaise with staff at all levels to follow up on any queries or concerns. To report service to service incidents using the Trust’s Risk Management Reporting system. To share service to service incidents with partner organisations where applicable. To receive and manage telephone queries from staff. To monitor and respond to email queries from the central team inbox. Amend any issues found, this includes anonymising any identifying information that has been recorded. Create additional records where multiple incidents are recorded within the one incident reported. Merge and triangulate records that have been reported multiple times. Reassign incidents where the handler/reviewer/area are incorrect. Complete incident validation sheets and email to relevant personnel. To produce regular reports to ensure quality checks are effective. Administrative Support To provide administrative support for the Risk Management Team. Act as first point of contact for telephone calls, being proactive in dealing with enquiries and taking messages. Provide administration support to the Risk Management Team, including booking venues, equipment, emailing relevant attendees with diary requests and additional documentation. Update incidents via the Risk Management Reporting System regarding dates of the Trust Patient Safety Event Review Panel, the outcome (scanning of documentation), named Investigators, dates reports are due and date of final sign off for investigations/review. Provide new templates when agreed at relevant panels within the Risk Management Software System. Archiving of documentation in accordance with the Trust Records Policy. Support the wider Quality & Safety Team with administrative duties. Risk Management Software Support Provide login and passwords for users of the Risk Management Reporting System. To provide reports when requested. Resolve any access issues from users of the Risk Management Reporting System. Escalate any unresolved issues to the InPhase Project Manager. Maintain the Risk Management Reporting System admin email inbox – actioning emails. Reviewing incidents that have been finally approved the previous day: To quality check for personal details that should not be in certain areas of the incident and make sure that the Learning from Patient Safety Events (LFPSE) sections have been completed correctly. To check all reported incidents daily to ensure level of harm is appropriately graded and then escalate to seniors if there is a concern. If further information is required within the incident, direct contact is made to the reporter of the incident to highlight the errors for future learning. Risk Management Reporting System Training Initial training for new starters on the Risk Management Reporting System. Ongoing training for all staff in reviewing incidents on the Risk Management Reporting System.
