Job summary The Universal Care Plan The Universal Care Plan Programme is a pan-London NHS service enabling every Londoner to have their End of Life care and support wishes digitally shared with the health professionals looking after them. Launched in July 2022, the UCP has expanded to include personalised care planning across a range of specialised patient groups e.g. Sickle Cell and CKD. The UCP programme team is small, compromising 7 team members across clinical and digital sections supported by two clinical Leads. The Universal care plan currently hosts 70,000 live care plans across the capital with the opportunity to grow in the future. Main duties of the job Your main focus will be to support, create and deliver a strategic plan for UCP completion across the borough, with a focus on community patients (not including those in Tertiary care organisations).
