Service:Hospital to Home
Salary: Scale 5 Point 12 £25,586 pro rata
Hours: 2 Positions available.
Part Time: 56 hours over the course of a fortnight. This averages at 28 hours per week and requires working alternate weekends. Current service hours are between 08:30am and 7pm but these may alter based on the needs of the service and funding.
Closing Date: Sunday 02nd February 2025 @ Midnight

About the role
In this role you will work as part of the Hospital to Home Team, who provide transportation to facilitate hospital discharge and who carry out support tasks within people’s homes to prevent further hospital admissions. The team, also, assess individuals’ wider support needs and effectively signpost and refer onto other organisations to ensure longer term support is put in place.
About the Hospital To Home Service
Our service prides itself on its “Home First” and patient-centred approach and supports those over 50, who have suffered a period of illness that has affected their independence following a hospital admission or visit to A&E. We work closely with professionals from across the Health & Social Care sector and a range of other agencies to encourage clients to access services that will improve their health and wellbeing and retain their independence.
This may involve activities such as a Pre-Discharge Home Check to ensure that the patient’s home is secure, warm and ready for their arrival and/or identifying potential support needs based upon each individual’s needs, such as hand rails/trip risks etc.
About you
You must have good communication and organisational skills and the ability to assess and respond to the needs of older people. Problem solving and using your initiative, whether lone-working or as part of a team, to provide support, you will work from our base at St James’ University Hospital and out in the community.
