We are looking for an enthusiastic and adaptable Urgent Care Service Assessor to join our strong and progressive team in the Folkestone area of South Kent Coast.

Main duties of the job
You will play a vital role in our multidisciplinary services; supporting patients with complex needs in the community. You’ll have the freedom to plan care in your own way, with time to develop your assessment and treatment skills with the support of senior staff.
Our aim is to build on collaborations so that we can continue to build a positive patient experience. As a short-term service, the predominant part of your role with be assessing, planning and reviewing care needs, liaising with other team members to ensure safe, high quality care.
You will be required to drive between patient homes and work sites and will need to provide your own transport to achieve this.

Qualifications
Essential criteria
1. Evidence of Foundation Degree Level study (Level 5) or equivalent (Nursing Associate). Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C or equivalent in English and Maths)

3. Minimum of 2 years’ experience as a Senior Healthcare worker Experience of working as part of a team Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E-roster / AT-P / Rio)
