Exciting opportunities have arisen to become part of an experienced and motivated Mental Health Social Care enablement service. These roles will support the delivery of mental health prevention and recovery, working with a strengths-based approach to enable adults with mental ill health to maximise their independence and connect to their community to ensure longer-term resilience.
The post-holders will work with people on a one-to-one basis and in a group setting, facilitating activities to support people to achieve their goals.
The roles will be responsive to the Adult Social Care Front Door and Mental Health Enablement Team and will shape and deliver person-centred care plans under the guidance of the Mental Health Enablement Coordinator.
Key Responsibilities:
1. Support the delivery of mental health prevention and recovery services.
2. Work one-to-one and in group settings with clients.
3. Facilitate activities to help clients achieve their goals.
4. Develop and deliver person-centred care plans.
