Responsibilities
Assist Care Managers in developing and implementing personalized care plans.
Coordinate services and resources for clients to ensure seamless care delivery.
Maintain accurate and up-to-date records of client interactions and progress.
Support clients and their families in understanding their care options and procedures.
Monitor client needs and report any changes to the Care Manager promptly.
Participate in team meetings and contribute to care strategy discussions.
Perform administrative tasks such as scheduling appointments and managing correspondence.
Requirements
High school diploma or equivalent; a degree in healthcare or social services is preferred.
Previous experience in healthcare, social work, or a related field is an advantage.
Strong interpersonal and communication skills to connect with clients and team members.
Ability to handle sensitive information with confidentiality and professionalism.
Excellent organizational skills and attention to detail.
Proficient in using computer systems and healthcare software applications.
Compassionate and empathetic mindset with a genuine desire to help others.

End Time: 17:00
Pay Per Hour: £18.66
